This form needs to be completed by organic producers if GM varieties are available in Canada for the conventional seed approved for planting. For example, there are genetically modified corn hibreds and soybeans varieties being grown in Canada.

This form also needs to be completed by processors if non-organic agricultural ingredients, food additives or processing aids are being used and there is a possibility that GM or GE materials may have been used in producing the ingredient, food additive or processing aid.

If you are a CSI client producing organic livestock or poultry flocks, you will need to complete this form or submit your own equivalent records.  This form needs to be updated annually with your recertification application.
